The Ultrastructural Study of the Thyroid Tumors

Abstract
The histopathogic diagnosis in thyroid gland tumors was confusing chiefly because of the wide variations in the gross and microscopic patterns. Follicular adenoma may be grossly and microscopically similar to follicular carcinoma, the distinction being made only by the findings of capsular or blood vessel invasion in carcinoma. Hyperplastic nodule with focal papillary or pseudopapillary fronds in the wall closely simulated the appearance of papillary carcinoma. To differentiate these morphologic similarities, electron microscopic studies of human thyroid glands diagnosed by follicular adenoma, follicular carcinoma, diffuse hyperplasia with focal papillary fronds, papillary carcinoma and anaplastic carcinoma were performed.
The results were obtained as follows;
1. Ultrastructure of follicular adenomas was characterized by columnar cells with microvilli, abundant mitochondria throughout cytoplasm and dense bodies in apical border of cytoplasm.
2. Follicular carcinomas, compared to adenomas, disclosed slight pleomorphic nuclei, cystically dilatated endoplasmic reticulum, a few of mitochondria and dense bodies, and less developed microvilli.
3. In the papillary carcinomas, nuclei were large, irregular with deep indentations at their periphery and most of them have characteristic intranuclear cytoplasmic inclusion with clear appearance. Well-developed microvilli and a few of mitochondria were noted in their cytoplasm. But dilated endoplasmic reticulum was not observed.
4. The pleomorphic spindle and giant cells of undifferentiated carcinoma showed cytoplasmic and nuclear characteristics similar to well differentiated papillary carcinomas. Irregular, indented nuclei with intranuclear cytoplasmic inclusion, abundant mitochondria and endoplasmic reticulum were noted. Nucleoli were prominent and nuclear body was observed.
